Dr. Gafanovich is a board-certified internal medicine physician who has cared for patients on Manhattan’s Upper East Side for more than 20 years. Every video visit is with Dr. Gafanovich herself, the same physician who sees patients at 1550 York Avenue.
Self-pay video visits are $275 for new patients and $175 for established-patient follow-ups.

New and established patients who are uninsured or whose insurance our practice does not accept can book directly on our website.
Patients must be 18 or older and physically located in New York State at the time of the visit.
Established patients with insurance accepted by our practice should contact the office to confirm telemedicine coverage and arrange their appointment.
Many concerns can be evaluated by video. If an examination or testing is needed, Dr. Gafanovich will explain the next step.
Prescriptions and refills are provided when clinically appropriate following a medical evaluation.
Previous medical records are required for all controlled-substance requests. Dr. Gafanovich will review these records, verify your current treatment, and check your prescription history in the New York State Prescription Monitoring Program (PMP/I-STOP) before considering a prescription.
Prescribing is at Dr. Gafanovich’s discretion and must comply with applicable state and federal requirements. A prescription is not guaranteed, and an in-person evaluation may be required.

Review of your progress, treatment, or testing performed through our practice.
These fees apply to eligible self-pay patients, including medical weight-management visits. Medications, laboratory testing, and any additional services are billed separately.
Insurance billing may be available only for established patients with an eligible in-network Blue Cross Blue Shield, Aetna, or Cigna plan. Coverage depends on your specific plan and the service provided. Applicable copays, deductibles, and coinsurance apply.
Please contact our office to confirm eligibility and arrange an insurance-billed visit. The self-pay fees above do not apply to covered visits billed through an in-network plan.
Can I start medical weight-loss treatment by video?
You can begin with a video consultation to review your medical history, goals, and treatment options. Dr. Gafanovich will determine whether medication is appropriate and whether laboratory testing or an in-person evaluation is needed. Medication costs are separate from the consultation fee.
Do I have to be in New York during the visit?
Yes. You must be physically located in New York State at the time of your appointment.

Can you prescribe medication by video?

Which video platform do you use?
We use doxy.me, a secure, HIPAA-compliant telemedicine platform. Our office will send you a link to join your visit.
Can I switch between video and office visits?
Yes. Patients can combine video consultations and follow-ups with office visits for examinations, laboratory testing, vaccinations, and other in-person care.
Is telemedicine appropriate for emergencies?
No. For a medical emergency, call 911 or go to the nearest emergency department.
