ADHD – Continuation of Care

This practice provides continuation of care only for patients with a previously established diagnosis of Attention-Deficit/Hyperactivity Disorder (ADHD). Care is focused on maintaining treatment continuity, monitoring response, and ensuring safe, appropriate follow-up.

ADHD care is provided within the context of an established physician–patient relationship and is guided by clinical judgment and applicable regulations.

ADHD Continuation of Care Policy

Continuation of ADHD treatment may be provided under the following guidelines:

  • Prior medical records documenting an established ADHD diagnosis and treatment history are required.
  • New diagnostic evaluations for ADHD are not performed by this practice.
  • Treatment is limited to continuation of existing therapy when clinically appropriate.
  • Ongoing care requires regular follow-up appointments for monitoring and reassessment.
  • Management is conducted in accordance with current clinical guidelines and applicable state and federal regulations.

The practice reserves the right to modify or discontinue treatment if clinical, safety, or regulatory considerations arise.

Frequently Asked Questions

Do you diagnose ADHD?
No. This practice does not perform new diagnostic evaluations for ADHD. Care is limited to continuation of treatment for patients with a previously established diagnosis.

What documentation is required?
