If you are applying for a green card from inside the United States, USCIS requires a medical examination by a designated civil surgeon. Dr. Marina Gafanovich is a USCIS-designated civil surgeon and you can verify her designation in the USCIS Civil Surgeon Locator and performs immigration medical exams in her Upper East Side office.
We complete the exam, the required tests and any missing vaccines in one place, and give you Form I-693 in a sealed envelope, ready to file.

A review of your medical history and a physical examination, including eyes, ears, nose and throat, heart, lungs, skin and lymph nodes, and a mental health assessment, as required by USCIS.

The civil surgeon checks your vaccination record against the CDC list for your age. Depending on age and history, it may include:
If you have no records, a blood test (titers) can sometimes prove immunity instead of giving the vaccine again. Any missing vaccines can be given in our office.
If a result needs follow-up (for example, a positive TB test), we explain the next steps and help you arrange them.

A civil surgeon is a doctor designated by USCIS to perform immigration medical exams inside the United States. Only a civil surgeon can complete and sign Form I-693.
Form I-693 must be filed together with Form I-485. Once filed, it stays valid for that application. Plan your exam close to your filing date.
