A chronic condition is not something you treat once. It needs a doctor who knows your history, watches the trends and adjusts treatment before small changes become big problems. What chronic care with us looks like

One doctor who sees the whole picture. Many patients have more than one condition and take several medications. Dr. Gafanovich reviews everything together, checks for interactions and keeps treatment as simple as possible.

Regular monitoring. Blood pressure, blood work, ECG and spirometry are done in our office, so you don’t need to go to several places.

A clear plan. You leave each visit knowing your numbers, your goals and what to do until the next visit.

Coordination with specialists. When you need a cardiologist, endocrinologist, gastroenterologist or other specialist, we refer you to trusted colleagues at NewYork-Presbyterian / Weill Cornell and stay involved in your care.

Our approach to specific conditions

Arthritis and joint pain. We evaluate the cause — osteoarthritis, inflammatory arthritis, gout or injury — with an exam and blood tests, and build a plan with medication, exercise and referrals to rheumatology or physical therapy when needed.

Back pain. Most back pain improves with the right combination of activity, medication and physical therapy. We look for warning signs that need imaging or a specialist.

Migraine. We identify triggers, prescribe treatment for attacks and, if migraines are frequent, discuss preventive medication.

Hepatitis. We screen for hepatitis B and C, monitor liver function and coordinate antiviral treatment with specialists. Hepatitis A and B vaccines are available in the office.

Pain management. We treat chronic pain with a careful, step-by-step approach that focuses on function and safety.

How often should I see the doctor?

Most patients with a stable chronic condition come in every 3 to 6 months. When treatment is being adjusted, visits are more frequent. Some follow-ups can be done by telehealth.
