Diabetes and Prediabetes Management

Type 2 diabetes develops slowly, often over years of insulin resistance and prediabetes. The earlier it is found, the more can be done — sometimes without medication at all. Dr. Marina Gafanovich diagnoses and manages prediabetes and type 2 diabetes on the Upper East Side.

Diagnosis

  • HbA1c — your average blood sugar over the past 2–3 months
  • Fasting glucose
  • Insulin resistance assessment when indicated
  • Kidney function, cholesterol and urine tests to check for complications

Treatment

Nutrition and activity. Practical changes that fit your routine and culture, not a one-size-fits-all diet.

Medication. Metformin and newer medications when needed, including GLP-1 and GIP/GLP-1 medications such as Ozempic (semaglutide) and Mounjaro (tirzepatide), which lower blood sugar and support weight loss. The doctor chooses the medication based on your health, other conditions and insurance coverage.

Protecting your heart, kidneys, eyes and nerves. Diabetes affects the whole body, so we also manage blood pressure and cholesterol, check your kidneys yearly and remind you about eye exams and foot care.

Regular monitoring. HbA1c every 3–6 months, with adjustments until you reach your goal.

Insulin resistance and prediabetes

Prediabetes means blood sugar is higher than normal but not yet diabetes. It is a signal, not a sentence: weight loss of 5–7% and regular activity can cut the risk of developing diabetes by more than half. We help you set up a plan and track progress.

Frequently asked questions

An HbA1c of 6.5% or higher on two tests means diabetes. Between 5.7% and 6.4% is prediabetes.

Often yes. Weight loss, regular exercise and changes in diet can bring blood sugar back to normal.
